All Medical ARMS forms must be completed before your scheduled Physical date. Any missing forms or documents could delay clearance and team participation by one week.
The following forms need to be completed before any team activities:
Physical must be completed by Mosaic Life Care Team Physicians
(Appointments will be scheduled by Team specific Athletic Trainer on campus)
We will not accept any outside physicals.
